CONSENT TO THE COLLECTION AND PROCESSING OF PERSONAL DATA

within the meaning of Act No.101/2000 Coll., on the protection of personal data and Regulation (EU) 2016/679 of the European Parliament and of the Council

 

By submitting this form and “clicking” on the option I agree, as a personal data subject, I give consent to the processing of my personal data listed below, which I hereby voluntarily provide to the data controller, in the manner set out below in this consent. I acknowledge that this personal data concerning me constitutes “personal data” within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (“GDPR”). I also hereby declare that I am at least 16 years of age and, if not, I confirm that this consent has been expressed or authorised by the person who exercises parental responsibility over me.

The data controller is SAMAK Services, s.r.o., Purkynova 74/2, Nove Mesto (Prague 1), 110 00 Prague, ID: 24291943, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 193829 (“Controller”).

 

  1. Processing of personal data2.1 My personal data that I voluntarily provide to the Data Controller is as follows: Name, Surname, E-mail, Telephone number.2.2 I consent to the processing of my above personal data for the purposes of:Marketing purposes of the Controller related to the offer of products and services; sending information about organised events, products, services and other activities (e.g. by sending newsletters, telemarketing); contacting for market research and marketing research purposes; contacting for the purpose of sending wishes for Christmas and Easter or other holidays and sending discount vouchers, gifts, etc;whereby the processing of personal data is necessary for the fulfilment of these purposes.2.3 The legal basis for the processing is Article 6(1)(a) GDPR.

    2.4 The controller processes personal data that is adequate, relevant and limited to what is necessary in relation to the purpose for which it is processed.

    2.5 In addition to the Controller, the following entities will collect and process the personal data referred to above:

  • processors of personal data: Poctivá agentura s.r.o. ID 09912983.

Recipients of personal data: The Administrator is authorized to transfer the personal data of the data subject in particular to the following recipients and categories of recipients: suppliers of the Administrator, employees of the Administrator, providers of transport services, providers of marketing and advertising services, financial institutions, state authorities within the framework of the fulfillment of legal obligations established by relevant legal regulations and other persons in a different contractual relationship with the Administrator.

2.6 The Controller is entitled to process the above personal data for a period of 5 years from the date of my consent. I acknowledge that before the expiry of this period, the Controller may contact me to renew my consent and that without renewal, my personal data will be permanently deleted after the expiry of the above period.

 

  1. Rights of the data subject3.1 I acknowledge that, as a data subject, I have the right to request from the Controller access to my personal data (Article 15 GDPR), their rectification or erasure (Article 16 or 17 GDPR), or restriction of processing (Article 18 GDPR) and the right to object to the processing (Article 21 GDPR). Under the conditions set out in Article 20 GDPR, as a data subject, I have the right to the portability of the personal data concerning me that I have provided to the Controller.3.2 As a data subject, I may contact the Controller regarding the processing of my personal data at the e-mail address info@samak.cz.

3.3. If I, as a data subject, believe that my personal data is being processed in violation of legal regulations, I have the right to contact the Controller with a request for redress. If the request is found to be justified, the Controller shall immediately rectify the defective situation. This is without prejudice to my possibility as a data subject to lodge a complaint directly with the Office for Personal Data Protection.

3.4 I hereby give my consent to the processing of my personal data by the Controller for the purposes, to the extent and for the period specified above. I acknowledge that the Controller will keep this consent on file for the purposes of fulfilling the Controller’s legal obligation to be able to demonstrate that I have consented to the processing being carried out.
